To extend the discussion of space debris being the potential causes of recent tragic event of the failure of STS-107 mission will be the main focus of this paper. Although space debris threats are known since the beginning of the space age, the orbital debris environment in LEO persistently become more hazardous due the space technology advancement. The tragic event of Columbia crash could potentially have been the result of artificial debris impact in hypervelocity.
